Indiana Youth Survey Reminder

The MSD of Martinsville will administer the Indiana Youth Survey to 6-12 graders on March 11, 2024. Parents/Guardians should complete this Indiana Youth Survey Parental Consent Form to consent or deny their child’s participation in the survey.
